Things The Patient Should Bring With Them At Their Appointment:
1. Completed health history and registration form (2 Sided)
2. Their dental insurance card (If they are not sure they have dental coverage they need to contact their human resource dept). & address and phone number of where to send insurance claims. Our office files claims as a courtesy to our patients, It's ultimately the patient's responsibility.
3. Any X-Rays or records they may have obtained from their previous dentist.
4. A form of payment.
